Summer GolF League membership costs

By submitting your information and joining the league, the following applies:

Assumption of Risk, Release and Indemnification Agreement

Whereas, I (applying person for membership), have requested permission from The Sump LLC to join the intramural golf league being played at the course located at Donald E Conroy Golf Course, 75 Cedar Street, Babylon, NY 11702 (the premises);

AND WHEREAS, I have been advised that such participation may involve the risk of injury through mis-hit balls, slip and falls, contact with other players, and/or other risks associated with the activity, but I still desire to participate;

NOW THEREFORE, for good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, and to induce The Sump LLC to grant permission to me to be allowed on the premises, and to be a participant in the league, to the maximum extent permitted by law, I, for myself, my heirs, my executors, my administrators, my successors and/or my assigns (collectively and individually, the releasors"),hereby: (1) hold harmless The Sump LLC from and against any and/or all loss, costs, claims, suits, damages and judgements(including attorney's fees and disbursements) of any kind or nature whatsoever, however caused, including but not limited to, those for property damage and bodily injury, including death, arising out of or in connection with my presence on or about the premises.

THE IS AN IMPORTANT PART OF THE LEAGUE. YOU ASSUME THE RISKS WITHIN AND BY SUBMITTING YOUR REGISTRATION AGREE TO THE STATEMENTS ABOVE.
